The platform uses a secure, patient-reported, online profile to fully describe the impact of a chronic condition. This profile allows patients to tell their story in enough detail to be meaningful and actionable for patients themselves, care providers, and researchers.
The Design Procces
With a tight deadline from our first client, we chose a lean design process that would lay the foundation for a scalable product. I led a series of meetings to identify our key personas, understand their needs, and sketch a minimum viable product to address them. User interviews, group white boarding sessions, stake holder meetings, invision prototyping, spec writing, development kick off meetings, design QA and user testing made up the core of my design process.
Patients can compare their data anonymously to the responses of other community members.
Community members can connect with other members whose experience matches their own. They can search and filter to find people based on what matters most to them. They can also receive recommendations for people to connect with based on their preferences, and they'll have access to groups and forums on a variety of topics. (In Development)
Patients can share as much or as little of their information as they choose with their medical provider after completing the profile.
Patients can stay up to date with information and insights chosen just for them, based on their online profile.
